~~Purpose: The purpose of this research was to develop an interdisciplinary evaluation questionnaire, using to evaluate physical education lesson plan for elementary school grade 1-3.
Methods: There are 2 processes in this research; Step 1 analyze qualitative data from documents and interviews by content analysis technique by using content analysis. Researcher created structural questionnaires to collect 5 specialists’ opinion who were selected specifically. Step 2 use interviewed data to design evaluation guidelines and search its quality.
Results: 1) According to qualitative data from documents and interviews by using content analysis technique, interview found that there are element, writing, study objectives, contents, study tools specification method, criteria, activities, media, and valued corrected related-evaluation tools. 2) Suitability of the evaluation form of integrated physical education management plan for interdisciplinary subjects, found that mean was 3.82 and standard deviation was 0.37. The result of the consistency of the evaluation form of learning management plan by experts it was found that all aspects of the consistency index were 0.96. Thus, these illustrate that this developed teaching evaluation form meets the criteria and can be used for collecting data.
Conclusion: The evaluation guidelines of infusion trans-disciplinary integrated physical education lesson plans Consistency meets the criteria and can be used to be data collection.

~~วัตถุประสงค์: การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อสร้างแบบประเมินผลแผนการจัดการเรียนรู้พลศึกษาเชิงบูรณาการข้ามวิชาแบบสอดแทรกของพลศึกษา
วิธีดำเนินการวิจัย: การศึกษาวิจัยแบ่งออกเป็น 2 ขั้นตอนดังนี้ ขั้นตอนที่ 1 วิเคราะห์ข้อมูลเชิงคุณภาพที่ได้จากการศึกษาเอกสารและการสัมภาษณ์ โดยใช้เทคนิคการวิเคราะห์เนื้อหา ผู้วิจัยได้สร้างแบบสัมภาษณ์แบบมีโครงสร้างไปเก็บรวบรวมความคิดเห็นของผู้เชี่ยวชาญ จำนวน 5 ท่าน ขั้นตอนที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาออกแบบแนวทางการประเมินรวมทั้งหาคุณภาพของแบบประเมิน ประเมินความเหมาะสมและความสอดคล้องของแบบประเมินที่ออกแบบและสร้างขึ้น
ผลการวิจัย:  1) จากการศึกษาเอกสารและวิเคราะห์ข้อมูลเชิงคุณภาพที่ได้จากการสัมภาษณ์ พบว่า แบบประเมินผลแผนการจัดการเรียนรู้พลศึกษาเชิงบูรณาการข้ามวิชาแบบสอดแทรก ในหัวข้อเรื่ององค์ประกอบ การเขียน จุดประสงค์การเรียนรู้ สาระการเรียนรู้ ระบุวิธีการวัดผลประเมินผล เครื่องมือที่จะใช้ในการสอน เกณฑ์การประเมินผล กิจกรรมการเรียนรู้ สื่อ และเครื่องมือที่ใช้ในการวัดและประเมินผล เกี่ยวกับการบูรณาการที่ทำให้มีคุณค่าและถูกต้อง 2) ความเหมาะสมของแบบประเมินผลแผนการจัดการเรียนรู้พลศึกษาเชิงบูรณาการข้ามวิชาแบบสอดแทรก พบว่า แบบประเมินผลแผนการจัดการเรียนรู้พลศึกษาเชิงบูรณาการข้ามวิชาแบบสอดแทรก มีความเหมาะสมอยู่ในระดับมากที่สุดมีค่าเฉลี่ยเท่ากับ 3.82 และ มีค่าส่วนเบี่ยงเบนมาตรฐาน  0.37  ผลความสอดคล้องของแบบประเมินผลแผนการจัดการเรียนรู้ โดยผู้เชี่ยวชาญ พบว่าทุกด้านมีค่าดัชนีความสอดคล้อง มีค่า 0.96 จึงถือได้ว่าแบบประเมินแผนการสอนฯ ที่พัฒนาขึ้นมีความสอดคล้องกันเป็นไปตามเกณฑ์ที่กำหนดไว้ สามารถนำไปใช้ในการเก็บข้อมูลได้
สรุปผล: แบบประเมินผลแผนการจัดการเรียนรู้พลศึกษาเชิงบูรณาการข้ามวิชาแบบสอดแทรกของครูพลศึกษาที่สร้างขึ้นมีความสอดคล้องกันเป็นไปตามเกณฑ์ที่กำหนดไว้ สามารถนำไปใช้ในการเก็บมูลได้
